Remove LanguageModelCache and improve TextDocument change

`LanguageModelCache` effect some bugs for `TypeScript` features.
So we need to remove this and Reduce reanalysis.

- [ ] remove `LanguageModelCache`
- [ ] Reduce reanalysis
  - [ ] Incremental update
