Proposal: Add CHANGELOG.md

Problem: Axiom currently lacks a standardized changelog file, making it difficult for users to track updates and for contributors to reference past changes.

Proposed Solution: Add a CHANGELOG.md file following the Keep a Changelog format to document releases, fixes, and new features in a structured way. This would improve transparency and make it easier to maintain release notes.
