Update UncheckedExtrinsic documentation #3592

Follow up on #2280
The introduction of TransactionExtension expands UncheckedExtrinsic beyond the previous signed/unsigned structure. The documentation around UncheckedExtrinsic is incomplete as there is no mention of new-school, general transaction types that it can represent. We need to update the mermaid diagram as well.
