Create a "New Member" Form

We would like a form for new members to complete upon joining OpenSMC. Information we would collect could include contact details, interests, skills, expected time commitment, etc.

• See the current draft.
• Make revisions and share the new form link in the Website Slack line channel. Request that the webmaster add this link to our Getting Started page.
