Authority-less MCP resources cannot be read by VS Code

1. Announce a resource like foo:///bar/baz.txt
2. Try to open it in VS Code
3. VS Code opens foo://bar/baz.txt

This is a limitation of our URI parser.

I added this

server.resource("myResource", "foo:///bar/baz.txt", async (uri) => ({
  contents: [{
    uri: uri.href,
    text: "App configuration here"
  }]
}));

and get this

Not sure whether I did something wrong
