Completion of error handling

Would you like to add more error handling for return values from functions like the following?

• pthread_cond_init ⇒ bioInit
• pthread_mutex_init ⇒ initServerConfig

This issue has been fixed. All failures are fatal and treated as such.

I suggest to avoid ignorance of return values a bit more.
Would you like to detect every error situation as early as possible?
