This pull request adds a derived description/mapping table for the waveforms data. The CSV file contains descriptions for waveform
label. Currently, there is no terminology standard for waveforms data. However, we did includeLOINCcodes for some concepts that are also present in the waveform numeric data table. The concepts were mapped to LOINC using the online tool SearchLOINC and Athena for OMOP codes. Descriptions and mapping codes were reviewed by @xborrat.
