This pull request adds a derived mapping table for medications. The CSV file contains
itemidtoomop_concept_idandomop_concept_codeforrxnormmappings codes. I manually mapped the RxNorm codes using RxNav and Athena. Mapping codes were reviewed and verified by @xborrat.
