utilnet: association properties and uwp icon

I hope it's not too late to add these small fixes, which I noticed during testing of popup and forms.

• UWP icon did not show up in Forms
• Forms design changed recently to match Popup design (i.e. no terminal prefix, association type checks, visible content, fraction along value)
• Popup not rounded, fraction on Maui not spanning rows
